Benefits An individual who is currently licensed in the state as a Registered Physical Therapist (PT). In collaboration with multi-disciplinary team develops a plan of care for each physical therapy patient and provides skilled physical services in accordance with physician orders. Essential Duties and Responsibilities: • Provides skilled physical therapy services/interventions in accordance with physician orders. • Assesses patient needs, plans for, evaluates, and modifies care to meet goals of physical therapy interventions. • Collaborates with all disciplines to plan and evaluate team goals for each patient. • Other duties as assigned Reflectx Services is a values based, full service healthcare employment organization that can find the job that you want, in the location of your choice, nationwide! We are currently seeking a PT. This individual will provide the physical, mental and social well being for families and individuals assessing, planning, implementing and evaluating the results. They will provide thorough and appropriate training to the patient and caregivers. Complete clinical records, schedules and administrative tasks. They will serve as a case manager and coordinator multi-disciplinary services. The work setting is going to be in the patient’s home, so travel is required. 1-year experience is required, and a RPT license in VA is required. Very competitive salary.
